Our primary goal is to assist you in attaining and maintaining optimal oral health.Your appointment time is reserved exclusively for you. We will respect your time and trust that you will do the same by arriving on time for your appointments. If it is necessary for you to cancel an appointment, we ask that you give us as much notice as possible so that your previously reserved time can be made available to another patient.

Our office requires at least 1 business days notice to reschedule your appointment unless you have become sick or must tend to a family emergency. Inconsiderate cancellations may be subject to a fee based on the time and procedure cancelled. Patients who consistently cancel or miss appointments without notice will not be rescheduled.

When you arrive for your appointment please have your health and insurance (if applicable) information available. If you are coming to us from a previous office you can facilitate the patient examination process by having your records (e.g., x-ray images, periodontal charting) forwarded to our office ahead of time.

Transferring Records

If you wish to transfer your records from our office, you may print out and utilize our records transfer form. Please note that you may incur a charge (not covered by third party dental insurance policies) for the transfer.
